Transition of Care For Members Enrolling in an Aetna Medical Plan on 1/1/202611/13/2025 Members currently enrolled in the Highmark Basic PPO and Geisinger Custom HMO plans and transitioning to an Aetna medical plan for 2026 may be eligible to request Transition of Care (TOC) coverage if currently receiving treatment from a provider who is not in Aetna’s network. You may qualify for TOC if you are in an active course of treatment for a serious condition such as pregnancy, cancer, or have a chronic illness requiring ongoing treatment. TOC allows you to continue seeing your current provider for a limited time—typically up to 90 days—while you transition to an in-network provider. To request TOC, you must complete and submit a Transition of Care Request Form within 90 days of joining the Aetna medical plan. You can find a copy the form by clicking on the link below.
